Whether a patient chooses medication, surgery, or both, they may experience changes to their sexual function.
The most common side effect to medication like alpha blockers, in treating BPH, is retrograde ejaculation. The man may notice that very little or no sperm is released during ejaculation. This is because the sperm travels backwards into the bladder instead of being released at the end of the penis. The sperm will then be expelled from the body when the man next urinates. Retrograde ejaculation is not harmful in any way and does not stop a man from reaching climax.
When surgery is used to treat prostate enlargement men may find the strength of their erection is weakened. This happens in around 5% of surgeries.
